Consent

I hereby waive any, all claims for damages against the JRDC, its members, its officers, or owners of any area where the JRDC's activities may take place, arising from my participation in any of its activities. I further agree to abide by all rules, regulations and policies of the JRDC and I understand that failure to comply with said rules will result in my suspension or expulsion from the JRDC, or removal from the activity. I hereby certify that the answers to the above questions are true and correct. I also fully understand that if any of the above questions have been answered falsely or incorrectly, my application will not be considered and/or my membership shall be revoked. I hereby apply to become an active member and if elected, agree to abide by all the rules, regulations and policies of the JRDC.
